Job summary

Cooper Norcross Health in Camden, NJ is seeking an Advanced Practice Provider (NP or PA) to deliver in-home post-discharge management for at-risk adults. The APP will perform histories and exams, develop care plans, order tests, and provide pharmacologic and non-pharmacologic therapies.

You will work autonomously or with supervising physicians, emphasizing health promotion and education within transitional care.

Qualifications

• Minimum of 3yrs, working with the adult and geriatric population as an Advanced Practice Provider.
• Current knowledge and understanding of Primary Care, Chronic Care Management, and Best Practices.
• Home based Primary Care experience preferred.

Responsibilities

• Perform patient history and physical examination to determine an individualized plan of care.
• Order, perform, supervise, and interpret diagnostic tests and prescribe therapies as appropriate.
• Provide teaching and counseling to individuals, families, and groups; work autonomously or with a supervising physician.
• Emphasize health promotion and disease prevention as part of comprehensive care.
