The HTCC collaborates with the care team to implement plans across the continuum, promoting efficient use of resources and patient satisfaction.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of a nursing program approved by a recognized accrediting agency.
• Eligibility for state licensure as a registered nurse in the United States.
• Foreign educated nurses require degree equivalency validation.
• Active RN license in a US state/territory or DC.

Responsibilities

• Coordinate and manage care for a defined patient panel via telehealth.
• Collaborate with the interdisciplinary team to implement a care plan.
• Promote access to timely care and optimize health outcomes.
• Provide leadership and guidance to staff in the program.
• Monitor quality indicators and participate in program evaluation.
• Educate staff on nursing processes and telehealth workflows.
