Summary

The Home Study Family Support Case Worker will develop individual child case plans and provide information, education, referrals, outreach, advocacy, and support. Maintain accurate case files and track progress. Assess safety and appropriateness of child placement for children in Juntos home study program with the goal to support family unification for immigrant children, family preservation, kinship care, and general child well-being.

Duties and Responsibilities:

  • Provide information, education, referrals, outreach, advocacy, and support to ensure that each child receives the services they require.
  • Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practices.
  • Develop individual case plans with particular attention to culture, language, and special circumstances.
  • Maintain accurate and complete case files by updating database systems, submitting reports, and managing physical case files.
  • Track individual progress via an online database.
  • Participate in regular training and communication sessions with on-site staff and caseworkers, supervisors and headquarters staff.
  • Maintain confidentiality in all areas of the service population and program operations.
  • Complete home study activities consisting of interviews, a home visit, and a written report containing the caseworker's findings.
  • Assess the potential minor sponsor's ability to meet the child's needs.
  • Educate and prepare the sponsor for the child's release and build on the sponsor assessment conducted by the care provider staff to verify or corroborate information gathered during that process.
  • Contribute to the organization's overall efforts to enhance permanency options for families by performing tasks in a timely manner; providing updates on a regular basis; observing the agency's work schedule and attendance policies; and following all agency regulations as outlined in the Employee Manual.
  • Meet or exceed program productivity expectations on a regular and sustained basis.
  • Ability to perform duties and services remotely from home.
  • Perform other duties as assigned by supervisor.

Requirements:

  • Bachelor's of Social Work (BSW) Degree or an equivalent degree in education, psychology, sociology, or other behavioral science equivalent.
  • Bilingual in English and Spanish.
