Job summary

Piedmont Fayette Hospital in Fayetteville, GA seeks a Home Medication History Information Technician (PRN) to assist the Home Medication Information RN in gathering accurate patient medication histories. You will support admissions by compiling comprehensive home med lists per policy.

Requirements include a HS diploma, 1 year retail pharmacy experience, and licensure/registration options for LPT and RPI; IV admixture and chemotherapy prep experience are preferred.

Qualifications

• HS diploma or GED required.
• 1 year of retail pharmacy experience required.
• Hospital pharmacy experience preferred.
• IV admixture and IV chemotherapy prep experience preferred.
• LPT license and RPI registration required upon hire; PTCB certification preferred.

Responsibilities

• Assist in obtaining the home medication history for admissions.
• Work under the Home Medication Information RN to ensure accurate med histories.
• Provide indirect patient care by following policy during the home med info process.
