Job summary

Jobtailor is seeking a Registered Nurse to perform in-home infusion therapy visits across patient plans of care, documenting clinical interventions, and collaborating with pharmacy teams. The role requires strong assessment and education skills, with experience in infusion therapies.

Responsibilities include patient education, care plan development, and liaison duties with families. Must hold an RN license, CPR/BLS, and 2+ years in home infusion; CRNI is preferred.

Qualifications

  • Graduate of an accredited nursing school.
  • Current RN state license.
  • 2+ years of experience in home infusion and infusion centers.
  • 2+ years of experience with IV Immunoglobulins and IV Biologics.
  • CRNI certification is preferred.
  • Experience with peripheral IV starts, infusion therapy, and phlebotomy for adult and pediatric patients is preferred.
  • Effective interpersonal, time management, and organizational skills.
  • Strong written and verbal communication skills.
  • Ability to maintain consistently accurate records.
  • Ability to direct information to the appropriate healthcare provider.
  • Proficient in computer skills, including word processing and efficient use of the internet and email.
  • Current CPR and BLS certification from the Red Cross or AHA.
  • Valid driver’s license

Responsibilities

  • Conduct in-home infusion therapy visits according to the plan of care.
  • Prepare clinical and nursing notes and document patient interventions in clinical and non-clinical records.
  • Act as a clinical resource and collaborate with pharmacy departments.
  • Maintain knowledge of drug side effects and interactions.
  • Provide telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures.
  • Ensure patients understand therapy plans, medications, and adherence requirements.
  • Serve as a liaison between patients, families, and the pharmacy.
  • Perform comprehensive initial and periodic head-to-toe assessments.
  • Initiate preventative and rehabilitative care.
  • Determine patient needs using assessment data and input from the agency team, physician, patient, and family.
  • Manage patient and family expectations regarding services, outcomes, and discharge goals.
  • Establish primary and secondary diagnoses based on patient assessment and home health care focus.
  • Develop care plans with skilled interventions, medical supplies, and necessary services.
  • Evaluate patient progress and revise care plans as needed.
  • Perform other duties as assigned
