Job summary

Enliven is seeking an experienced RN to service home infusion and infusion center patients in West Jordan and surrounding locations. The nurse educates patients and families, performs admissions, case management, and follow-up visits, and collaborates with physicians and pharmacies to develop treatment plans.

Candidates should have an Associate or Bachelor degree in Nursing, five years of nursing experience (acute care preferred), CPR/BLS required with ACLS/PALS preferred, and strong IV/vascular

Qualifications

• RN with five years nursing experience, preferably in acute care.
• Strong Med/Surg, ICU, ER, acute care experience.
• Home Health/Infusion experience preferred.
• Current CPR/BLS required; ACLS/PALS preferred.
• Ports/CVC/PICC access and IV placement skills.

Responsibilities

• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions and follow physician and Clinical Manager directions.
• Confer with physicians and pharmacies to develop initial treatment plans.
• Implement care plans and educate patients and families.
• Report patient care, conditions, and progress to physicians andClinical Manager.
• Discharge patients after consultation and complete documentation.
• Prepare and update patient records, care plans, and reports.
