Job summary

Qualifications

• Active CA RN license in good standing.
• At least 1 year of infusion/ER/ICU RN experience.
• Valid CA driver’s license and proof of insurability.

Responsibilities

• Provide direct patient care in home health setting within RN scope.
• Assess, diagnose, plan, implement, and evaluate patient care with family involvement.
• Collaborate with physicians and interdisciplinary team to develop care plans.
• Facilitate start of care and admission processes.
• Advocate for patient rights, safety and preferences.
• Document changes and communicate with team and physicians promptly.
• Educate patients/families on infusion therapy management and self-care.
