Job summary

Enliven Home Infusion Specialists Arizona is seeking an experienced Registered Nurse to service home infusion and infusion center patients in Provo and surrounding locations. The ideal candidate will be well-trained, compassionate, and able to provide high-quality nursing care with minimal supervision.

This role requires strong Med/Surg, ICU, ER experience, IV placement skills, and proficiency with EMR systems.

Qualifications

• Associate or Bachelor Degree in Nursing.
• Minimum of five years of nursing experience preferred in an Acute Care setting.
• Strong Med/Surg, ICU, ER, acute care experience.
• Home Health/Infusion experience preferred.
• Current CPR/BLS required.
• ACLS; PALS preferred.
• Skilled in accessing Implanted Ports; Central Lines; PICC lines.
• Strong IV placement skills.
• Strong organizational and communication skills.
• COVID-19 vaccination preferred.

Responsibilities

• Complete daily electronic forms for each patient in the EMR system.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care and infusions and follow physician orders.
• Develop initial treatment plans with physicians and Clinical Manager based on orders and patient assessment.
• Provide hands-on care and teaching under physician guidance.
• Report patient care updates to physician, pharmacy, and Clinical Manager.
• Implement care plans with patient and family to achieve optimal outcomes.
• Discharge patients after consultation with physician and Clinical Manager with proper documentation.
• Document case conferences, patient contacts, medication changes, and care plan updates.
• Prepare visit/shift reports and coordinate with other healthcare disciplines.

Job Summary

Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Provo and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care.

Responsibilities

• Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System.
• Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients.
• Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager.
• Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation.
• Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously.
• Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
• Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.

Job Requirements

• Background check required

• Associate or Bachelor Degree in Nursing Minimum of five years of nursing experience preferred in an Acute Care setting
• Strong Med/Surg, ICU, ER, acute experience
• Home Health/Infusion experience (not required, but preferred)
• Current CPR/Basic Life Support certifications (BLS) required
• Advanced Cardiac Life Support (ACLS); Pediatric Advanced Life Support (PALS) (preferred)
• Skilled in accessing Implanted Ports; Central Lines; PICC lines
• Strong IV placement skills
• Strong organizational and communication skills
• Preferably COVID-19 Vaccinated (some pharmacy partners & patients require a vaccinated nurse)
