Job summary

RPh on the Go USA, Inc. seeks a Home Infusion Staff Pharmacist for Irving, TX on a 13-week contract. The role runs 7:00 AM to 3:30 PM, focusing on sterile compounding and home infusion therapy.

Responsibilities include evaluating eligibility, developing therapy plans, preparing and dispensing sterile meds, and educating patients and caregivers. The ideal candidate holds a PharmD, has sterile IV experience, MTM or immunization background, and knowledge of pharmacokinetics.

Qualifications

• Doctor of Pharmacy (PharmD) or graduate of an accredited School of Pharmacy.
• Minimum one year of sterile IV compounding experience.
• Experience with non-sterile compounding, immunizations, and MTM.
• Completion of an ASHP-accredited Pharmacy Residency.
• Completion of an Advanced Pharmacy Practice Experience (APPE).
• Equivalent pharmacy experience.

Responsibilities

• Evaluate patients for home infusion therapy eligibility.
• Develop, monitor, and adjust medication therapy plans using clinical assessments and labs.
• Prepare, verify, and dispense sterile medications per USP standards and regulations.
• Educate patients, caregivers, and providers on therapy and infusion administration.
• Collaborate with providers to ensure continuity of care and monitor outcomes.
• Manage infusion pumps and equipment, ensuring safety and QA.
• Provide drug information and maintain documentation to support compliance and safety initiatives.
