Job summary

KabaFusion is seeking a licensed pharmacist to provide safe, comprehensive services in a home infusion setting. You will assess therapy, dispense medications and equipment, document patient progress, and collaborate with care teams to ensure high-quality outcomes.

Ideal candidates have a PharmD or BS in Pharmacy, active state licensure, and 2+ years in acute care or home infusion, with IV admixture experience. Join a patient-focused team delivering expert care across multiple states.

Qualifications

• Pharmacist with a PharmD or BS in Pharmacy and active state license.
• Minimum two years acute-care or one year in home infusion experience.
• Experience with intravenous admixture services and IV therapy preferred.

Responsibilities

• Provide safe, appropriate pharmacy services in home infusion settings.
• Assess therapy appropriateness and monitor patient progress toward goals.
• Document communications and maintain patient medication profiles.
• Collaborate with physicians, nurses and other healthcare professionals.
• Oversee staff and ensure compliance with USP standards.
