Option Care Health is seeking a Home Infusion Nurse II to coordinate and provide intermediate direct patient care in the home or an infusion suite in Colchester/ Norwich, CT. This full-time role supports patient safety under a physician plan of care and requires travel as needed.

Responsibilities include administering treatments, teaching patients and families, and timely documentation for billing and care continuity. Strong nursing skills in infusion and home care are essential.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Current CPR certification required.
• 1–2 years of infusion nursing experience preferred.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care in the home or infusion suite.
• Assesses patient needs and administers medications and education to patients and families.
• Communicates with clinical team and documents care in real time for billing and continuity of care.
