Job summary

RPh on the Go is seeking a Clinical Pharmacist in Syracuse, NY to join our home infusion team. The role supports clinical expertise in a home infusion setting with a consistent Monday through Thursday schedule from 8:00 AM to 5:00 PM.

The ideal candidate will hold an active New York pharmacist license, have 2+ years acute care or 1 year home infusion experience, and be proficient in IV admixture and reviewing patient therapies for safety and efficacy. CareTend familiarity is a plus.

Qualifications

• Current licensure as a Registered Pharmacist in NY state is required.
• Minimum of two years recent acute care experience or at least one year in home infusion preferred.
• Strong knowledge in intravenous admixture services and intravenous therapy.
• Ability to tailor drug therapy for home care patients in collaboration with multidisciplinary teams.

Responsibilities

• Review infusion orders and patient therapies for safety and efficacy.
• Participate in clinical rounds to guide patient care strategies.
• Perform sterile compounding and TPN review following best practices.
• Maintain detailed patient records and individualized care plans.
• Ensure compliance with USP standards and cleanroom protocols.
• Train and support staff to enhance team performance.
• Lead operational improvements within the infusion service.
