Job summary
Option Care Health in Bakersfield, CA is seeking a Clinical Pharmacist (Part-time/Per Diem) to provide safe and effective pharmacy services in the home care setting. You will coordinate with nursing, educate patients and caregivers, and monitor pharmacotherapy outcomes.
Responsibilities include evaluating patient suitability, designing care plans, dispensing medications, and ensuring compliance with DEA and state regulations. Requires active State Pharmacy license and Pharm.D. degree.
Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: 1 year of sterile IV admixture experience or related qualifications.
Responsibilities
- Determines patient suitability for home care and collects patient information.
- Designs and monitors patient-specific pharmacy care plans for home care.
- Provides medication-use education to staff, patients and caregivers.
- Ensures safety, quality assurance and regulatory compliance in dispensing.
