Job summary

Residential Home Health and Hospice, serving the Chicago area, seeks an experienced RN for end-of-life hospice care at patients’ homes. You will join an interdisciplinary team delivering compassionate, patient-centered care with autonomy and support.

Two years of RN experience (hospice preferred) and a valid driver's license are required; travel within the assigned territory is expected. A strong EMR workflow supports documentation and coordination of care.

Qualifications

• Active RN license in the state you work
• Two years of RN experience; hospice experience preferred
• Valid driver’s license; willingness to travel ~20% of the workday

Responsibilities

• Deliver patient-centered care in a hospice setting with an interdisciplinary team
• Perform comprehensive assessments, symptom and pain management, and end-of-life care planning
• Educate and support patients and families through the end-of-life journey
• Maintain accurate EMR documentation
• Work independently within a team and travel to patient homes within territory
