Job summary

Kindful Health LLC is seeking a Part Time Hospice RN to administer nursing care for patients in their homes and coordinate with the interdisciplinary team. You will travel locally to patient homes and participate in care planning and education.

The role requires RN licensure in the state, at least 2 years of clinical nursing experience, and strong communication skills in English. Travel and day shifts are expected.

Qualifications

• Education: Graduate from an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Licensure: Current RN license and ability to drive.
• Experience: 2 years RN experience in clinical care; hospice preferred.
• Skills: Good interpersonal skills and English reading/writing.
• Transportation: Reliable transportation and insurance.

Responsibilities

• Assess patient needs across physical, psychosocial, and environmental domains.
• Coordinate admission and ongoing plan of care with the IDG/Team.
• Educate patients and families on disease process and end-of-life care.
• Provide nursing procedures and preventive care as assigned.
• Communicate changes to physicians and team; document progress.
• Support on-call responsibilities and after-hours care as needed.
