Schedule Details/Additional Information:
Typical work hours are Monday to Friday, 8:00 AM to 4:30 PM, with weekend and holiday rotation.
Sign-On Bonus:
$2,500 for external hires
Major Responsibilities:
- Contribute to effective patient outcomes by providing competent patient care.
- Demonstrate competent home health aide skills.
- Assist patients in reaching functional status goals by providing support with ADLs, following the HHA assignment record, and ensuring safety.
- Observe and report changes in patient’s condition, inform the case manager or RN/Supervisor, and document accordingly.
- Understand and embrace the hospice philosophy, providing specialized care to terminally ill patients.
- Perform personal care, feeding, laundry, light cleaning of patient rooms, and washing.
- Ensure patient safety using equipment properly and safely.
- Take temperatures, pulse, respiration, and blood pressure as directed.
- Provide skin care and reposition patients to prevent breakdown.
- Change patient bed linens and clothing as requested by the patient and family.
- Maintain consistent work activities and exhibit the ability to work independently.
- Attend to personal work schedule professionally and consistently.
- Work cooperatively with team members.
- Handle issues related to death and dying with calmness and professionalism.
- Communicate effectively with all customers.
- Participate in case conferencing and IDGs to achieve patient outcomes.
- Document patient care accurately, adhering to agency, state, federal, and accreditation standards within required timeframes.
- Maintain patient confidentiality in accordance with HIPAA and privacy laws.
- Participate in CQI processes and quality improvement projects.
- Implement cost-effective measures in patient care.
- Identify problems and contribute to solutions.
- Ensure patient satisfaction and adhere to regulatory requirements.
- Complete annual in-service hours, hospice education, and OSHA requirements.
- Support scheduling of supervisory visits and provide services according to regulatory standards.
- Meet all training requirements mandated by the state and HIPAA.
- Demonstrate professionalism, flexibility, and teamwork.
- Identify personal developmental needs and contribute to learning plans.
- Maintain home health aide skills independently and support organizational goals.
Education/Experience Required:
- High school diploma or equivalent; one year of nurse’s aide experience; RN nursing students meeting state requirements.
Knowledge, Skills & Abilities:
- Recognize patient needs and behaviors across age groups, understand child development and aging, and communicate effectively in English.
- Must be certified as a nursing assistant in Illinois.
Physical Requirements and Working Conditions:
- Travel within service boundaries in various weather conditions and road types; assist with patient transfers up to 200 pounds.
- Make visits to private residences, which may not be accessible to individuals with disabilities.
- Ability to lift up to 35 pounds unaided; use patient handling equipment for heavier lifts, with assistance as needed.
