Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team in Massachusetts. You will provide skilled nursing and rehabilitative care to clients at home, enabling them to remain in independent living and assisted living communities such as Centerville, Hyannis, Yarmouth and Brewster.

Expect to perform head-to-toe assessments, triage urgent situations, manage acute and post-acute care needs, and document with point-of-care tools.

Qualifications

• A current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited nursing program.
• At least one year of hands-on medical experience with adults/-geriatric populations in relevant settings.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify subtle changes in patient condition.
• Utilize experience with acute and post-acute care (CHF, COPD, wounds, IV therapy, post-surgical care).
• Triage and independently manage urgent clinical situations in the home.
• Conduct medication reconciliation, patient teaching, and safety monitoring.
• Perform wound assessment, care, and documentation.
