Job summary

CHI Franciscan Health at Home is seeking a skilled Speech Language Pathologist to assess and treat patients in home settings, tailoring plans to each individual's needs and collaborating with families and health teams for optimal outcomes.

The role requires a Master’s in SLP, an active state license with CCC-SLP, CPR/BLS certification, and at least one year of clinical speech therapy experience. A valid driver’s license is also required.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active registered SLP license in the state where you practice with CCC-SLP.
• CPR/BLS certification.
• One year of Speech Therapy experience in a clinical setting.
• Current driver's license.
• Must comply with federal, state and local laws and regulations.

Responsibilities

• Provide therapy to patients in their homes, including techniques to improve speech, language, cognitive-communication, and swallowing skills.
• Customize therapy strategies to meet individual patient goals and objectives.
• Utilize AI-powered clinical documentation and best practices to optimize care delivery and documentation.
• Provide training and support for family members to assist with exercises at home.
