CenterWell Home Health, a Humana Inc. company, seeks a Speech-Language Pathologist to deliver home-based speech therapy and rehabilitation services. You will evaluate patients, develop care plans, and coordinate with care teams to improve communication, swallowing, and overall quality of life.

You will work under CenterWell Home Health services, with opportunities for ongoing education and patient-family counseling, while maintaining documentation and compliance with company policies.

Qualifications

• CCC-SLP/CFCC credential required.
• Minimum of six months experience as a Speech-Language Pathologist.
• Home Health experience a plus.
• Current and unrestricted license.
• Current CPR certification.
• Good organizational and communication skills.
• Valid driver’s license, auto insurance, and reliable transportation required.

Responsabilités

• Evaluate and provide speech-language pathology services at the patient’s home or facility.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications including audiologic screening.
• Document changes and report to supervisor and physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit required documentation and reports.
• Prepare timely evaluations, visits, care plans and progress reports.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and follow company policy.
• Attend staff development programs and continuing education.

Connaissances

ASHA CCC- or CFCS credential

Minimum 6 months SLP experience

Home Health experience

Current and unrestricted license

CPR certification

Organizational skills

Driver's license and transportation
