Job summary

Atrinity Home Health, part of LHC Group, is seeking a Speech Therapist to assess and treat speech and language disorders in home health settings. You will develop treatment plans, perform interventions to improve communication, swallowing, and cognitive function, and evaluate progress with the care team.

Responsibilities include conducting functional assessments, collaborating with nurses and physicians, educating patients and families, and documenting outcomes.

Qualifications

• Licensed in Speech Therapy in the state of residence.
• Current CPR certification.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Evaluates the patient's function using diagnostic/prognostic tests and assists in care plan development.
• Treats patients to improve communication and swallowing abilities.
• Monitors patient response to treatment and reports changes to the supervising clinician.
• Educates patient, family and caregivers on speech therapy activities and participation.
