South Charlotte/Union County, NC

The Home Health Speech Therapist will provide medically prescribed speech therapy services to patients in their homes. The therapist will work with patients who have communication and swallowing disorders as well as cognitive and language impairments. The therapist will develop treatment plans in collaboration with the treating physician, monitor progress, adjust treatment as needed, and educate patients and their families on speech therapy techniques and exercises.

Responsibilities

• Evaluation and functional assessment of the patient and establishment of a plan of care.

• Coordinate care, including care management and discharge planning as needed, with patient/responsible party, physician, other disciplines, and/or external resources.
