Job summary

Pathways Healthcare is seeking a Speech-Language Pathologist to provide home health and hospice services in Stoneham and surrounding areas. The role focuses on patient-centered evaluation and therapy under the supervision of physicians and collaboration with care teams.

Candidates must hold a Master’s in Speech-Language Pathology, ASHA CCC, and active licensure in NH and ME, with at least one year of clinical experience and CPR certification.

Qualifications

• Master’s degree with internship from an approved program in Speech–Language Pathology.
• ASHA Certification (CCC) for clinical practice.
• Licensed to practice Speech–Language Pathology in NH and ME.
• Minimum of 1 year of clinical experience in Speech–Language Pathology.
• Strong verbal and written communication and organizational skills.
• Current CPR certification.
• Licensed driver with reliable vehicle.

Responsibilities

• Conduct evaluations and assess home environment for speech-language needs.
• Identify functional speech defects and plan care to improve patient function.
• Administer speech therapy programs using specialized techniques/equipment.
• Recommend communication devices/aids as indicated.
• Coordinate with referring physicians and care teams for comprehensive care.
• Participate in case conferences, in-services and other required meetings.
• Prepare and submit notes per policy.
• Maintain clinical competency in speech-language pathology practice and theory.
• Identify patient/family needs for other services and refer as appropriate.
• Instruct patient and caregivers in treatment procedures as indicated.
• May supervise home health aides related to speech-language plan of care.
• Performs other duties to facilitate service delivery.
