Pathways Healthcare is seeking a Speech-Language Pathologist to join our home health and hospice team. You will conduct evaluations, develop plans of care, and deliver therapy in patients' homes across New Hampshire and surrounding areas.

We leverage AI-assisted documentation to save clinicians time (5–10 hours per week) while keeping clinical judgment at the center. Per-visit compensation, mileage reimbursement, and ongoing training support await you.

Qualifications

• Master’s degree in Speech-Language Pathology with internship.
• CCC-SLP certification per ASHA requirements.
• NH and ME speech-language pathologist licensure.
• Minimum 1 year clinical experience in Speech-Language Pathology.
• Good verbal and written communication, and organizational skills.
• Current CPR certification.
• Licensed driver with insured vehicle.

Responsabilités

• Conduct evaluations and assess home environment for speech/language needs.
• Identify speech defects and establish a plan of care to improve function.
• Administer speech therapy programs using specialized techniques and equipment.
• Recommend communication devices or aids as indicated.
• Collaborate with referring physician and staff to coordinate comprehensive care.
• Participate in meetings to ensure coordinated care.
• Prepare and submit notes per organization policy.
• Maintain clinical competency in practice and theory.
• Refer patients to other services as appropriate.
• Instruct patient and family in treatment regimens.
• May supervise home health aide in related procedures.
• Perform other duties to deliver services.
