Apollo Home Healthcare is seeking a Speech-Language Pathologist to provide in-home visits across Contra Costa, Alameda, and Tri-Valley areas. This W-2 per diem role offers flexible scheduling, autonomy, and meaningful patient relationships within a supportive team.

Required: Master’s in Speech-Language Pathology, active CA SLP license, and CPR certification. Home health experience is a plus; travel and reliable transportation are necessary.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Active California SLP license required.
• Current CPR certification required.
• Home health experience preferred but not required.
• Excellent documentation and communication skills.

Responsibilities

• Perform speech-language and swallowing evaluations in patients' homes.
• Develop and implement individualized Plans of Care.
• Provide skilled speech therapy interventions.
• Advise on communication devices and adaptive equipment as needed.
• Assess home environments for safety and communication needs.
• Collaborate with physicians, nurses, PT/OT, and care teams.
• Complete timely clinical documentation.
• Educate patients and caregivers on exercises and strategies.
• Supervise home health aides when applicable.

Apollo Home Healthcare is seeking a Speech-Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County, Alameda County, and the Tri-Valley area. This is a W-2 per diem role ideal for SLPs looking for schedule flexibility, autonomy, and meaningful patient relationships. You'll provide high-quality, one-on-one care to patients in their homes while collaborating with a supportive interdisciplinary team.

What You’ll Do

• Perform speech-language and swallowing evaluations in patients' homes
• Develop and implement individualized Plans of Care
• Provide skilled speech therapy interventions
• Recommend communication devices and adaptive equipment as needed
• Assess home environments for safety and functional communication needs
• Collaborate with physicians, nurses, PT/OT, and care teams
• Participate in case conferences as appropriate
• Complete timely, compliant clinical documentation
• Educate patients and caregivers on exercises and compensatory strategies
• Supervise home health aides when applicable to the plan of care
• Active California Speech-Language Pathologist license
• Master's degree in Speech-Language Pathology
• Current CPR certification
• Home health experience preferred (but not required)
• Strong documentation and communication skills
• Ability to work independently in the field
• Reliable transportation and valid driver's license
• $110-150/visit + $45/hour for training
