Job summary

Gurwin Healthcare System in Commack, NY seeks a Speech Language Pathologist for its Home Care Division on a per diem basis. The role covers Nassau County and Eastern Suffolk with travel to patients' homes.

You will assess, diagnose, and treat speech, language, and dysphasia needs, provide patient and family training, and ensure timely documentation and billing. Per visit compensation is listed with mileage reimbursement.

Qualifications

• Licensed NYS Speech-Language Pathologist
• Minimum of two years experience
• Home health experience preferred

Responsibilities

• Plan and provide speech/language and dysphasia rehabilitation services to patients in their homes.
• Provide training and/or assistive devices for speech-language rehabilitation to patients and families.
• Report initial status, plans and goals, ongoing status, and discharge plans to the agency.
• Complete timely documentation including assessments, plans, goals, visit care, summary reports, and billing data.
• Instruct patients and families and team members on programs to enhance care and follow-up.
