Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities. You will help patients regain speech, swallowing, and communicative abilities, while participating in care planning and documentation.

You will work under the Clinical Manager, ensuring high standards of professional conduct, patient confidentiality, and coordination with families and healthcare staff.

Qualifications

• Certification of Clinical Competence in Speech Language Pathology or Audiology per ASHA.
• Minimum six months of experience as a speech therapist/speech language pathologist.
• Current and unrestricted state license.
• Current CPR certification.
• Valid driver’s license and auto insurance with reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in development/review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction to patients in alternative communication systems when appropriate.
• Maintain and submit documentation and written reports as required by policy.
• Participate in care coordination and discharge planning.
