Job summary

AuthoraCare Collective in North Carolina seeks a PRN Speech-Language Pathologist Case Manager for our Home Health department serving Alamance and Rockingham Counties. The role requires travel to patients' homes to provide services per the plan of care under the primary care physician.

You will assess, develop and implement a home exercise program, document progress, collaborate with the IDT, and educate patients and families.

Qualifications

• Master’s or doctoral degree in speech-language pathology.
• Minimum of one year of speech therapy experience; home health or facility preferred.
• Current and unrestricted Speech-Language Pathology licensure.
• Current CPR certification.

Responsibilities

• Provide speech-language therapy in accordance with the patient’s plan of care.
• Assess and develop individualized home exercise programs.
• Perform comprehensive assessments including cognition, swallowing, and communication deficits.
• Manage the therapy plan for a caseload of Integrated Health patients in collaboration with the IDT.
• Establish goals and interventions; collaborate with physicians to implement treatment plans.
• Educate patients and families and document goals and outcomes of care.
• May perform evaluations and treatments for hospice patients as needed.
