Job summary

CenterWell Home Health is seeking a PRN Home Health Speech Language Pathologist to provide in-home evaluation and treatment for adult and senior patients. You will assess speech, language, cognition, and swallowing disorders; develop and implement individualized care plans; and deliver evidence-based therapies in patients' homes.

You will collaborate closely with an interdisciplinary team, educate patients and caregivers, document visits accurately, and support safe, independent living.
