CenterWell Home Health is seeking a PRN Speech Language Pathologist to provide in-home evaluation and treatment for adult and senior patients. You will assess speech, language, cognition, and swallowing disorders and develop individualized care plans.

In collaboration with an interdisciplinary team, you will educate patients and caregivers, document visits accurately, and support safe, independent living while delivering evidence-based therapies in patients' homes.

Qualifications

• Licensed Speech-Language Pathologist (State license required).
• Experience in home health or adult populations preferred.
• Ability to work independently in patients' homes.
