Job summary

Jobtailor is seeking a dedicated Speech-Language Pathologist to deliver in-home or facility-based services. You will evaluate, treat, and monitor communication, voice, language, hearing, and swallowing disorders, and develop Plans of Treatment and Care with attention to patient progress and safety.

Responsibilities include diagnostic testing, data collection, family instruction, and collaboration with care teams to ensure quality patient outcomes and proper documentation.

Qualifications

• Meet education and experience requirements for ASHA Certification of Clinical Competence in Speech-Language Pathology or Audiology
• Minimum six months of experience as a speech therapist or speech-language pathologist
• Current and unrestricted license
• Current CPR certification
• Valid driver's license
• Auto insurance and reliable transportation
• Maintain vehicle insurance meeting residing-state minimum limits or specified Humana limits, whichever is higher
• Home Health experience preferred
• Willingness to complete tuberculosis screening if selected

Responsibilities

• Evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities
• Participate in developing and periodically reviewing Plans of Treatment and Plans of Care
• Assess and treat disorders of speech, voice, language, hearing, and swallowing
• Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening
• Observe, record, and report patient condition changes and treatment responses
• Instruct patients on alternative communication systems when appropriate
• Counsel and instruct patients, families, and healthcare staff
• Maintain and submit required documentation
• Prepare written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports
• Participate in care coordination and discharge planning
• Maintain confidentiality and disclose information only when authorized and in the patient's best interests
