Job summary

Adaptive Home Health, LLC seeks a Speech-Language Pathologist (SLP) for full-time, field-based work in Georgetown/Hutto, TX. You’ll perform in-home evaluations, deliver skilled visits, and drive outcomes with a clear care plan while coordinating with the care team and caregivers.

You will be supported by a clinical/operational team and work with productivity-based compensation. Homecare Homebase is used for documentation and scheduling tools are being rolled out to streamline routing and visits.

Qualifications

• Active Texas SLP license in good standing.
• Strong clinical reasoning and field independence.
• Comfort escalating changes to RN/MD.
• Excellent caregiver education and teach-back skills.
• Reliable execution of orders and patient goals.
• Valid driver's license and daily travel ability.

Responsibilities

• Own outcomes: SLP plan, progression, and functional results.
• Perform in-home comprehensive SLP evaluations and visits.
• Assess and treat aphasia, cognitive-communication issues, and dysphagia screening.
• Develop and implement a measurable plan of care.
• Coordinate with multidisciplinary team and orders.
• Coach patients/caregivers with practical routines.
• Document promptly in Homecare Homebase (HCHB).
