Job summary

Freedom at Home is seeking a Speech-Language Pathologist (SLP) to provide in-home therapy on a per-diem basis. You will evaluate and treat patients with speech, language, cognitive, and swallowing disorders, and educate caregivers.

Qualifications include a Master’s or Doctoral degree in SLP, active state license, CCC-SLP preferred, and CPR certification. Home health experience is preferred. Valid driver's license and reliable transportation required.

Qualifications

• Must hold a Master’s or Doctoral degree in Speech-Language Pathology from an accredited program.
• Current valid state licensure as an SLP.
• CCC-SLP credential preferred when applicable.
• CPR certification is required.

Responsibilities

• Evaluate patients with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans based on patient needs and physician orders.
• Provide skilled speech therapy services in patients' homes.
• Educate patients and caregivers on communication strategies, swallowing safety, cognitive exercises, and home programs.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Complete accurate, timely clinical documentation and participate in case conferences and quality improvement initiatives.
• Promote safe, compassionate, high-quality care that helps patients remain independent in their homes.
