Job summary

Creative Solutions Services, LLC is seeking a Speech-Language Pathologist for a home health, 13-week contract in Monroe, North Carolina. The role is onsite with 8:30–5:00 shifts, Monday through Friday, serving geriatric adults with conditions such as CVA, TBI, and dysphagia.

Requires state licensure, CCCs, and a Master’s degree or higher, plus 2+ years of experience in home health. This position offers field autonomy with support from an interdisciplinary team and a structured orientation.

Qualifications

• Master's degree or higher required.
• State license and CCCs (post CFY).
• BLS, driver’s license, insurance and vehicle required.
• 2+ years of experience; home health experience favored.

Responsibilities

• Assess and treat swallowing difficulties and dysphagia.
• Develop and evaluate care plans; identify aspiration risks.
• Educate patient and family; determine diet modifications.
• Collaborate with an interdisciplinary team; schedule and document visits.
