Home Health Scheduling Coordinator - Vero Beach, FL

Activa Home Health (an AccordCare Company) is an accredited home health agency seeking an experienced full-time Scheduling Coordinator in our Vero Beach Office. The Scheduling Coordinator will work with the Medicare Division and must be familiar with Medicare guidelines. They will schedule clients based on referral information and authorizations.

Responsibilities
  1. Schedule and coordinate all field staff to meet patient needs, in accordance with the Plan of Care.
  2. Work closely with the Clinical Manager to manage patient schedules.
  3. Keep the computer system accurate and up-to-date with schedule changes and visit authorizations.
  4. Communicate with Intake, referral sources, sales staff, and team members to facilitate patient care.
  5. Answer phones and manage office supplies.
Qualifications

Experience

Minimum of 1 year experience in Home Health (required).
