Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team, delivering skilled nursing and rehabilitative care to clients in Northbridge, Upton, Hopedale, Milford, Holliston and surrounding towns. You will play an integral role in helping clients remain at home with compassionate, high-quality care.

Qualifications

• Massachusetts nursing license required or eligible to practice in MA.
• Graduation from an accredited nursing program, with official transcripts or diploma.
• At least one year of hands-on medical experience with adult/geriatric populations in acute care, home health, hospice, or rehab settings.

Responsibilities

• Perform comprehensive head-to-toe assessments and identify changes in patient condition.
• Triage and manage urgent clinical situations in the home setting.
• Conduct medication reconciliation, patient education, and safety monitoring.
• Perform wound assessment, care, and documentation.
