Job summary

UnitedHealth Group is seeking a Registered Nurse to provide and direct nursing care in patients' homes, coordinating comprehensive care with physicians and the care team. The role includes supervising home health aides and licensed nurses, performing supervisory visits, and ensuring documentation meets policy and regulatory standards.

The RN will assess patients (including OASIS), reconcile medications, and collaborate with physicians, patients, and families to ensure continuity and high-quality

Qualifications

• Current, unrestricted RN licensure in state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire (preferred).
• 1+ year of Home Health experience preferred.

Responsibilities

• Coordinate total plan of care with physicians and health care professionals.
• Supervise home health aides and licensed nurses and provide instruction.
• Perform supervisory visits and ensure compliance with policies and regulations.
• Complete OASIS assessments, medication reconciliation, and timely documentation.
• Communicate with physicians, patients, families, and agency staff to ensure quality care.
