Job summary

Optum’s Louisiana Home Health division is seeking a Registered Nurse to provide and direct home-based nursing care per physician orders. You will collaborate with the care team to coordinate the total plan of care and ensure continuity for patients in their homes.

The role requires an unrestricted RN license in Louisiana, a valid driver's license, and strong communication skills. Compensation includes a comprehensive benefits package, with a $5,000 sign-on bonus and competitive per-visit and

Qualifications

• Current and unrestricted RN licensure in Louisiana.
• Valid driver’s license and access to a reliable vehicle or transportation.
• CPR certification or ability to obtain within 90 days of hire.
• 1+ year of Home Health experience preferred.

Responsibilities

• Develop and revise the plan of care in collaboration with the physician and health care team.
• Supervise home health aides and licensed nurses, provide instruction, and assign tasks.
• Provide supervisory visits and ensure compliance with regulations and policy.
• Document visits, complete OASIS assessments, and maintain accurate patient records.
