Qualifications

Responsibilities

• Provide comprehensive assessments of bio-psycho-social needs; perform skilled nursing care and prescribed treatments; educate clients and families according to plan of care.
• Develop and document individualized care plans with the health team; communicate patient status and progress.
• Maintain effective communication with office staff; coordinate services and referrals.
