Devotion Hospice is seeking a Home Health Registered Nurse to deliver patient-centered care in patients' homes within the Scottsdale area. You will perform comprehensive assessments, develop and implement care plans, educate patients and families, and coordinate with interdisciplinary teams to promote recovery and independence.
Ideal candidates hold an active RN license, CPR certification, a valid driver's license, and prefer at least 2 years in Home Health.
Formación
- Must hold an active RN license.
- Valid driver’s license.
- CPR certification.
- Valid auto insurance.
- At least 2 years of Home Health RN experience preferred.
- HomeCare HomeBase experience preferred.
Responsabilidades
- Perform initial and ongoing comprehensive assessments.
- Develop, implement, and document the plan of care.
- Educate patient and family on self-care and nutrition.
- Coordinate care with interdisciplinary team.
- Determine service scope based on acuity and needs.
- Provide infection control and standard precautions.
- Direct Home Health Aide and LPN/LVN as needed.
- Monitor progress and communicate changes to physicians.
