Job summary

• Current CPR certification.

Responsibilities

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Ensure services are medically necessary and documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
• Maintain accurate, timely, and compliant documentation within the EHR.
• Take part in Medicaid audits, quality assurance initiatives, and case conferences.
• Champion patient needs and help connect them with community resources and support services.
