Qualifications

• Licensure/Certification: IL or MI RN Licensure depending on work location.
• Current American Heart Association HealthCare Provider BLS required prior to start date.
• Provides own transportation and maintains valid driver's license.

Responsibilities

• Collaborates with RN Case Manager to implement home health plan of care.
• Provides evidence-based, patient-centered care at patients' residence.
• Communicates with care team members to facilitate patient outcomes.
