Responsibilities

The Home Health Patient Care Manager is responsible for supervising and coordinating clinical services, providing and directing nursing care to patients in their homes as prescribed by physicians, in compliance with applicable laws, regulations, and agency policies.

1. Provide clinical services within the scope of practice, as defined by state laws, in accordance with the care plan, and in coordination with the healthcare team.
2. Receive referrals, assign clinicians appropriately, evaluate patient orders, and coordinate start of care visits.
3. Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
4. Ensure patient needs are continually assessed, and care is individualized, appropriate, and meets home health eligibility criteria, following physician orders.
5. Review assessments and care plans daily, consult with clinicians as needed, and provide recommendations.

Education and Experience

• Current RN licensure in the state of practice.
• Current CPR certification.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
