Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to administer skilled care in the home setting. The role involves teaching patients and families and aligning services with physician orders and the care plan.

The position is part-time, 24 hours/week over four days, with eligibility for our 60% paid days off program. Candidates must be licensed in their state, a graduate of an approved clinical education program, and have at least one year of licensed professional experience.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year experience as a licensed professional.
• Medicare home health or hospice experience preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
