Position Summary

The registered nurse is accountable for the delivery and supervision of safe, quality care that is individualized for their assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation). She/He is responsible for the assessment, treatment and care of patients in populations and ages as assigned. The registered nurse accepts and promotes professional development as an integral part of nursing practice. She/He will access, analyze, customize, coordinate and communicate the patient\'s plan of care and activities collaboratively with other members of the health care team. Families and or significant others will be included within the plan of care as appropriate. She/He will direct, delegate and manage the care delivered by colleagues and subordinates. The registered nurse performs all duties in a manner that respects and supports the family centered care model. Collegial and collaborative care will reflect the philosophy of the Division of Nursing, the Mission and Core Values of the Sisters of Providence, and National Patient Safety Standards.

Position Specific Responsibilities
  • Possesses knowledge of developmental needs and competent in the assessment and treatment as it relates to the age of the patient population served.
  • Conducts and documents a comprehensive patient assessment upon admission.
  • Continually reassesses and applies appropriate advocacy and interventions as indicated.
  • Continently interprets a patient\'s self-assessment and behavior with consideration of cognitive, physical, emotional, psychosocial and maturation factors.
  • Actively involves patient and family in the collaborative plan of care.
  • Demonstrates and promotes safe patient care practices (e.g., hand washing, patient ID, infection risk reduction, medication safety & reconciliation, falls prevention, no preventable deaths/injuries, inclusion of patient/family concerns).
  • Collaborates with the patient, family and healthcare team to develop, document, implement and monitor an individualized plan of care based on best practice and evidence-based medicine.
  • Provides, documents, and consults to ensure patient and family education supports the treatment plan.
  • Adaptable to change and updated patient care practices within an evolving healthcare environment.
  • Collaborates with team members, patient, family and others in the development of a proactive discharge plan.
  • Performs procedures and treatments according to department/unit protocols, guidelines, standards and the Nurse Practice Act.
  • Legibly and electronically documents patient care activities in an accurate, concise, timely manner.
  • Competent in input and retrieval of information within written and electronic documentation.
  • Provides safe, quality care in an efficient, cost-effective manner.
  • Supports and assists with orientation of new staff and students.
  • Assumes preceptor and charge nurse responsibilities when selected and functioning in those roles.
  • Advances professional nursing through continuing education, required training, certification, and participation in department and unit activities.
  • Ensures competencies, certifications, safety information and courses are completed as required.
  • Identifies concerns and involves colleagues and management in promoting a safe patient care environment.
  • Engages in positive communication, feedback, and ongoing development of self and team.
  • Promotes open communication, patient- and family-centered care, and healthy collegial relationships.
  • Demonstrates adaptive/timely communication based on patient/family needs.
  • Attends or reviews minutes for a minimum of 50% of scheduled staff meetings and remains accountable for meeting content.
  • Models collaborative practice with other health care professionals.
  • Recognizes the role of cultural diversity in relationships with patients, families, staff, and community members.
  • R rounds with physicians and team members to promote continuity of care and patient confidence in the care delivery team.
  • Monitors patient condition, notes changes, uses judgment and takes appropriate action.
  • Communicates patient status using SBAR format with colleagues and physicians; uses quality and safety tools during handoffs.
  • Utilizes Providence Early Assessment Team (PEAT) when indicated by changes in patient condition or concerns.
  • Promotes comfort and pain management measures.
  • Documentation reflects assessment, treatment and re-assessment to meet patient goals.
  • Demonstrates safety and infection control principles as defined by policy, National Patient Safety Goals, and emergency procedures.
  • Complies with hospital standards to ensure regulatory requirements are met (e.g., JCAHO, OSHA, state and federal requirements).
  • Participates in departmental and unit activities, including quality improvement programs and processes.
  • Participates in departmental activities to improve patient care and process systems; participates in tracer rounds and assists others in applying knowledge clinically and operationally.
  • Safeguards all forms of confidential information related to patients, families, staff and the organization.
  • Adheres to organizational policies regarding fraud and abuse, conflict of interest and code of conduct.
Education and/or Experience
  • AK: Required - Graduate of an accredited/approved school of Nursing.
  • AK: Preferred - 1 year RN experience in an acute care hospital (unless hired as an intern or fellow).
Skills
  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure; compute rate, ratio and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business materials; write reports, and effectively present information; strong written and verbal communication.
  • Other Skills and Abilities:
    • Ability to understand, read, write, and speak English. Aligns with integrated healthcare at home philosophy.
    • Schedule varies to meet department needs.
    • Ability to prioritize multiple tasks and work with diverse health unit teams.

Physical Demands and Work Environment

The demands of this role require the ability to perform essential functions with reasonable accommodations as needed. Regular standing, walking, and manual dexterity are required, with ability to lift and move objects up to 50 pounds. Visual acuity and color/depth perception required.
