AdvisaCare is seeking a compassionate Home Health RN to provide one-on-one nursing care in patients' homes, develop care plans, and educate families to promote independence. You’ll collaborate with the care team and document visits using our EMR, supporting patients through recovery and chronic condition management.

Active RN license and strong clinical judgment are required; home health experience is a plus, with training available for the right candidate.

Qualifications

• Active RN license.
• Strong clinical judgment and a caring, patient-centred approach.
• Home health experience is a plus, but not required — we’re happy to train the right nurse.
• Comfort with technology and electronic documentation.
• A positive attitude, great communication skills, and a genuine love for helping others.

Responsabilités

• Provide one-on-one nursing care in the comfort of patients’ homes.
• Complete assessments, develop care plans, and deliver high-quality clinical care.
• Educate patients and families to support independence and confidence.
• Collaborate with physicians, therapists, and your care team to ensure seamless care.
• Document visits accurately and timely using our user-friendly EMR.
• Support patients through recovery, chronic condition management, and health maintenance.
