Job summary

CenterWell Home Health is seeking a dedicated Home Health Registered Nurse to join our caring team. You will provide one-on-one skilled nursing in patients' homes, manage care plans with physicians, and help patients regain independence.

Reporting to the Clinical Manager, you will collaborate with a team of clinicians, educate families, monitor progress, and ensure accurate documentation. This patient-centered role requires a valid RN license and current CPR certification.

Qualifications

• Med surg, ICU, ER, or acute experience.
• Current and unrestricted Registered Nurse licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes, including meds administration and wound care.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medication, and treatment options.
• Maintain accurate records and coordinate with other healthcare professionals.
• Report patient care and condition progress to physician and Clinical Manager.
• Oversee Home Health Aides and monitor their appropriate completion of documentation.
