Qualifications

Must hold an RN license and nursing degree.
Experience in Med-Surg, ICU, ER, or acute care.
Current CPR certification.
Experience collaborating with healthcare professionals.
Valid driver's license and reliable transportation.

Responsibilities

Provide skilled nursing care to patients in their homes, including medications and wound care.
Develop and implement individualized care plans with physicians.
Monitor patient conditions and report changes.
Educate patients and families on disease management and treatment options.
Maintain accurate patient care records and coordinate with the care team.
Oversee Home Health Aides and monitor documentation.
