Job summary

Interim HealthCare Inc. is seeking a compassionate Home Health Registered Nurse (RN) to provide personalized, one-on-one in-home care across Roanoke and surrounding areas.

You will assess patients, develop care plans, and coordinate with physicians and the care team to improve outcomes. This role offers flexible scheduling (FT/PT/PRN), competitive per-visit pay, and mileage reimbursement, with opportunities for growth and ongoing training within a supportive, patient-focused organization.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in Virginia.
• Minimum 2 years of nursing experience with hands-on patient care, wound care, and blood draws.
• CPR certification and TB test required.
• Knowledge of home health regulations.
• Strong communication and critical thinking skills.
• Independent and patient-focused with compassion.

Responsibilities

• Provide home-based nursing care to patients recovering from illness or surgery.
• Conduct patient assessments and monitor progress.
• Develop and implement individualized care plans.
• Educate patients and families on medications, treatment plans, and home safety.
• Collaborate with physicians and care team members.
• Supervise LPNs, CNAs, and aides involved in patient care.
• Document patient progress and report changes to physicians.
• Coordinate discharge planning and ensure patient goals are met.
• Maintain accurate and timely documentation.
