Interim HealthCare Inc. is seeking a compassionate Home Health Registered Nurse (RN) to provide one-on-one, in-home care across Lynchburg and surrounding areas.

The role offers flexible scheduling (Full-Time / Part-Time / PRN) with competitive per-visit pay and mileage reimbursement. You will assess patients, create care plans, educate families, and coordinate with the care team to achieve optimal outcomes.

Qualifications

• Active RN license in Virginia or state of practice.
• Minimum of 2 years nursing experience; home health preferred.
• CPR certification and TB skin test required.

Responsabilités

• Provide home-based nursing care to patients recovering from illness, injury, or chronic conditions.
• Conduct patient assessments and monitor progress.
• Develop and implement individualized care plans.
• Educate patients and families on medications, treatment plans, and safety at home.
• Collaborate with physicians and interdisciplinary care team members.
• Supervise LPNs, CNAs, and aides involved in patient care.
• Document progress and report changes to physicians.
• Coordinate discharge planning and ensure patient goals are met.
• Maintain accurate and timely documentation.
