Job summary

hhccareers is seeking a Registered Nurse (RN) to provide skilled nursing care to patients in their homes and community settings. The role focuses on completing OASIS assessments, developing individualized care plans, and collaborating with interdisciplinary teams to ensure high-quality, outcome-based care.

Responsibilities include performing comprehensive OASIS evaluations, coordinating with physicians and therapists, supervising LPNs and aides, maintaining documentation, and supporting care

Qualifications

• Graduate of an Associate Degree or Diploma Nursing Program.
• Current RN license in the State of Connecticut.
• Minimum of one year of nursing experience or one year in a specialty nursing area.
• Strong computer skills.

Responsibilities

• Complete comprehensive OASIS assessments and evaluations.
• Review referrals and initiate patient/family contact.
• Develop and implement individualized plans of care with patients, families, physicians, and care team members.
• Perform OASIS assessments throughout the episode of care to ensure consistency and quality outcomes.
• Provide skilled nursing visits, assess patient progress, and evaluate responses to treatment.
• Educate patients and caregivers on disease management, treatment plans, and health promotion.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Supervise LPNs and Home Care Aides in carrying out the established plan of care.
• Maintain accurate and timely clinical documentation and reporting requirements.
• Participate in care conferences, interdisciplinary team meetings, SBAR reports, and quality improvement initiatives.
